One is a substance, the other is a preparation
Most entries in this atlas are names of substances. They have a formula, a molecular weight range, and two suppliers offering material under the same name are generally offering the same thing. The nucleotide-derived class belongs on that side.
Exosomes do not. The name refers to an object defined by how it was made. Two products can both be described as exosomes while differing in source cell, in isolation method, and in whether they were characterised at all. So the sentence “an exosome study reported this result” attaches, on its own, to no product. Three variables have to be checked first.
The evidence tiers are not equivalent
Reading the two entries at the same weight is a misreading. In this atlas the nucleotide-derived class has a randomised study cited; the exosome side has reviews only. No randomised controlled trial of an exosome preparation in a facial aesthetic indication was found in the literature searches behind this atlas, and the review literature in the field describes the technology itself as early-stage.
