Energy modalities and the tissue response
This document is technical information about delivery routes. It does not recommend or evaluate any procedure, product or device.
Question
How do energy-based devices stimulate collagen compared with injected materials?
Energy-based devices deposit heat rather than a substance. The literature describes controlled thermal injury followed by a wound-healing response, with collagen and elastin changes reported afterwards. This ends in the same place as the injected biostimulators — new matrix laid down by the host — but nothing is left in the tissue, so duration, revision and risk are governed by different variables.
Delivery routes — one axis: is a channel created?
| Route | Stratum corneum | Reported depth | What the literature reports |
|---|---|---|---|
| Non-ablative radiofrequency | Heating of the dermis and layers beneath is reported | Current passes through tissue and heat is generated by resistance. Surface cooling protects the epidermis while deeper layers are heated selectively. A systematic review of face and neck rejuvenation assessed effectiveness and safety across clinical studies. 34923652 | |
| Microneedle radiofrequency | Set by needle length; energy is delivered at the needle tips | Insulated needles place energy below the epidermis, so heating is concentrated at depth rather than at the surface. A critical review notes that how this compares with other energy modalities remains unclear from the published evidence. 33577211 | |
| High-intensity focused ultrasound | Focal zones are reported in the dermis and below | Acoustic energy converges at a focal point, producing discrete zones of thermal coagulation while the tissue above is left intact. Laboratory work in aged skin reported increases in collagen and elastin fibre synthesis with a described molecular pathway. 37759497 | |
| Ablative fractional laser | Channel depth varies with the settings used | Tissue is vaporised to leave vertical channels surrounded by zones of thermal damage. A review correlating device classes with histology summarises what is observed at tissue level for this and other modalities. 35165220 | |
| Non-ablative fractional laser | Reported to reach the dermis while the epidermis is spared | Energy is absorbed in the dermis while the stratum corneum is left unbroken, producing columns of thermal effect without an open channel. The histologic review above covers this class alongside the ablative devices. 35165220 |
This axis is not a ranking. For the methods marked as preserving the barrier, leaving the epidermis intact is the design objective.

Limitations & open questions
Device settings, tip design, pass count and cooling differ between systems and between operators, so a histologic or clinical finding reported for one system does not describe the energy class. This page names energy categories rather than equipment because a claim attached to a particular device would be a claim about a product this reference does not evaluate. One review cited here states explicitly that how radiofrequency microneedling compares with other energy modalities is not settled by the published evidence.
Nothing is left behind
The injected biostimulators in this reference work by leaving a particle the body responds to. Energy leaves nothing. Heat is deposited, the tissue responds, and the response is the whole of the effect. That difference decides which questions apply: there is no residence time to ask about, no material to remove, and no lot-to-lot variability — but equally no way to modify the stimulus after the session ends.
The axis that organises this page
Whether the stratum corneum is broken separates these modalities into two groups, and it is the same axis used on the delivery routes page. It matters for a different reason here: an open channel changes the recovery course and the infection consideration, while an intact barrier is the design objective of the modalities marked as preserving it.
Why no equipment is named
Findings in this field are generated on specific systems with specific settings. Attaching them to a named device would make this a product claim, which this reference does not make on any page. Energy category, depth and what the literature reports at tissue level are the level at which a statement can be carried between studies.
